Reports to: Senior Family Support Worker
Employment Status:  Non-Exempt/Full Time
Salary Range: $33,000 – $40,000

Qualifications:

  • Bachelor’s in Human Services or related field (Required)
  • At least 1 year experience working with children and families.

Job Responsibilities:

  • Maintain a caseload of 10-12 children.
  • Ensure that all releases are obtained.
  • Explain and provides on-call information to families on their caseloads.
  • Maintain regular contact in person ad by phone with the child, their parents, and the school.
  • Provides reward-based recreation activities.
  • Participate in the weekly unit assessment team meetings.
  • Develop a service plan that addresses the issues presented at intake.
  • Collaborate with other staff to exchange pertinent information.
  • Coordinates family team meetings with unit staff and other service providers
  • Maintain the case record by writing progress notes, developing the YASI plan, and ensuring that releases and respite consents are in the file folder.
  • Facilitates respite placements as part of the service plan.
  • Make requests for recreation and wrap around funds to assist in meeting service goals.
  • Responsible for the on-call coverage for one week on a rotating basis.
  • Attend CFLR staff meetings and other agency functions as required.
  • Provide for the occasional transportation of program clients in accordance with CFLR’s transportation policy.
  • Attend monthly meetings of CFLR PINS program staff.
  • Abide by NASW Code of Ethics.
